If your child(ren) require the administration of medication during school hours and/or will have medication held in the school office, please complete the forms below and submit them to the office in September. A Medication Management Form and one or both of the Authorization Forms is required to be completed.
Please note: prescription medication requires the signature of the prescriber/physician.
