If your child(ren) require medical management at school, including the administration of medication during school hours or medication to be stored in the school office, please note that forms are not posted online.

To receive the appropriate medical management documentation for your child, please visit the school office in person or email windsor@epsb.ca. Medical details are required in order to determine which forms are necessary, and providing forms directly helps ensure accuracy and avoids potential errors.

Please note:

• A Medication Management Form and one or more Authorization Forms may be required.

• Prescription medication requires the signature of the prescriber/physician.‌
